Open ElderGuardian on their Apple Watch and enter their first name and age. Allow the Health permissions when asked — without heart rate and heart rate variability the app cannot do anything useful.
On the monitored person's iPhone, choose This is my household, then tap Invite a caregiver and send the link however you like. When they tap it, their readings start appearing on that person's phone.
Heart rate variability differs enormously between healthy people, so there is no useful default. The app learns the wearer's own resting range before it will judge anything against it — about 40 readings across at least a day. Falls and extreme heart rates are watched from the first minute.
